Hypertension managment

The Hypertension Disease Management Program is designed for members who have hypertension and provides them with information about hypertension and lifestyle issues. This information includes topics such as: causes, treatment, nutrition, exercise, medications and more. This program does not replace the direction and guidance of the member’s healthcare practitioner, however it is intended to compliment and reinforce the information received in the office. The goal of the program is to provide tools to educate members on promoting improved health through prevention, detection, treatment and education.

How do I refer a patient?

Sanford Health Plan identifies eligible members through claims, Case Management, Worksite Wellness screenings and also encourages practitioners to send referrals to our Disease management programs. By completing and returning the referral form to Sanford Health Plan, you are partnering with us to provide the best possible care to our members - your patients.

May is National High Blood Pressure Education Month

According to the National Institutes of Health, High blood pressure affects about 50 million--or one in four--American adults. Of those with hypertension, about 68 percent are aware of their condition--but only 27 percent have it under control. The reasons for this include not taking drugs as prescribed and/or not taking a medication that sufficiently lowers blood pressure.

This year's theme highlights the threat of uncontrolled hypertension. The National Heart, Lung, and Blood Institute (NHLBI) urges Americans: "If Your Blood Pressure Is Not Lower Than 140/90, Ask Your Doctor Why."
