
The Hypertension Disease Management Program is designed for members who have hypertension and provides them with information about hypertension and lifestyle issues. This information includes topics such as: causes, treatment, nutrition, exercise, medications and more. This program does not replace the direction and guidance of the member’s healthcare practitioner, however it is intended to compliment and reinforce the information received in the office. The goal of the program is to provide tools to educate members on promoting improved health through prevention, detection, treatment and education. How do I refer a patient?Sanford Health Plan identifies eligible members through claims, Case Management, Worksite Wellness screenings and also encourages practitioners to send referrals to our Disease management programs.
