Healthy Pregnancy

Program Goals

Sanford Health Plan provides maternity care benefits from prenatal through postpartum care. We also encourage our pregnant mothers to join the Health Pregnancy Program.

The Healthy Pregnancy Program focuses on education and awareness. The main objective is to assist a member in identifying concerns early so she and her healthcare practitioner can takes steps to prevent or minimize any problems and ensure a healthy pregnancy.

The American College of Obstetricians and Gynecologists (AGOG) guidelines state that medical, obstetric and lifestyle factors can complicate a pregnancy. Through regular prenatal care, women can take action to increase their chance of having a healthy baby. The member’s healthcare practitioner continues to be the primary caregiver who provides the member with medical services and advice.

Enrollment

Eligible members for the Healthy Pregnancy Program are identified as female Sanford Health Plan members of all ages who are pregnant. Members are identified through Utilization Management when contact is made with the Plan to notify us of their pregnancy and obtain maternity care benefit information. Program Content

The content of the program was designed to compliment the care members receive from their healthcare practitioner. Program participants are asked to complete a health risk assessment which is reviewed for risk factors. Members are then stratified as low, moderate or high risk based on risk factors identified in the health risk assessment and/or telephone calls made by Quality Resource RN throughout the program.

Customized educational packets are sent to members. These packets contain educational materials and tools that compliment their situation and personal preferences. Subject matter includes: prenatal care and testing, nutrition, exercise, preterm labor, fetal development, postpartum depression, newborn screenings and vaccinations, car seat safety and more. A welcome baby packet is sent to members after they deliver and includes a breast feeding guide, infant nutrition information and more.

Sanford Health Plan is responsible for adopting and distributing clinical practice guidelines for acute, chronic and behavioral health care services that are relevant to our membership and recommends the following clinical practice guidelines for pregnancy management:
